PARENT/GUARDIAN AGREEMENT AND ACKNOWLEDGMENT
I understand that the Foundations Summer Session is an educational enrichment and tutoring program operating Monday through Thursday from 9:00 AM to 12:00 PM.
Drop-Off and Pick-Up
I understand that student drop-off is between 9:00 AM and 9:10 AM and pick-up is between 11:50 AM and 12:05 PM. I agree to arrive within these designated times. Students will only be released to a parent/guardian or an individual listed as an authorized pickup person.
Health and Emergency Care
I certify that the information provided on this registration form is accurate. In the event of a medical emergency, I authorize the tutoring center staff to contact emergency medical services and seek appropriate medical care for my child if I cannot be reached immediately.
Behavior Expectations
I understand that students are expected to behave respectfully toward staff, fellow students, and property. Repeated disruptive or unsafe behavior may result in dismissal from the program without refund.
Personal Property
The tutoring center is not responsible for lost, stolen, or damaged personal belongings brought to the program.
Snack Policy
I understand that my child may bring a small snack and drink each day. I acknowledge that while reasonable efforts will be made to maintain a safe environment, the tutoring center cannot guarantee an allergen-free facility.
Release of Liability
I understand that reasonable supervision will be provided during program hours. In consideration of my child's participation in the Foundations Summer Session, I voluntarily assume the ordinary risks associated with participation in educational activities and release and hold harmless the tutoring center, its owners, employees, and volunteers from claims arising from ordinary negligence, except where prohibited by law or resulting from gross negligence or intentional misconduct.
No refunds or make-up sessions will be provided for missed days due to illness, vacations, scheduling conflicts, or other absences.
